With bank draft, 1/12 of the annual dues will be deducted from your account at the beginning of each month. Payment by bank draft will automatically renewed each year unless PAGE is notified, in writing, 30 days prior to cancellation date.

To apply for bank draft, please complete our Membership Application indicating that you would like automatic bank draft and return the application to the address listed at the top of the form. Please be sure to include a voided check from the account PAGE Foundationthat is to be deducted and a check to cover your first month's dues. The check should be for the amount of monthly dues indicated below, depending on the level of coverage required. If you wish to join your local chapter, please include a check for full payment of the local dues amount. We cannot deduct local dues with bank draft.
